We accept the following health insurances. The list may not be all inclusive because of the continuing changes with employer provided insurance and insurance companies. Please call the office if you do not see your insurance company listed here. The patient is responsible for paying all deductibles and co-pays at the time of service. If you are unsure of your deductible or co-pay, please call your insurance company before your appointment to find out how much your co-pay and deductible is and how much of the deductible you have met.
